The Kavli Trust Programme on Health Research uses SurveyMonkey for our surveys. To do this we sometimes need some to store personal data and we need your consent to do this.

It is the Norwegian foundation Stiftelsen Dam (Akersgata 28, 0158 Oslo, Norway) that is managing the programme on behalf of The Kavli Trust and is the data controller.

The personal data described below is collected to be able to evaluate our application and review processes and the impact these processes have on the applicants.

What personal data we collect

When you get a survey from us (with a link to this page) via SurveyMonkey your email address and your name will be stored with your answer. In addition we sometimes ask for:

Age group
Education/degree
Address
Place of work
Title

Why we need the information and when it will be deleted

This personal data is collected for us to be able to get feedback on our processes and to improve them. Some of the information in the surveys will be used for research and development (R&D) projects and might be connected to data in SurveyMonkey Apply (our application and review software). The analyses might be a part of future scientific publications. In this case data will be anonymised and personal data will not be included without written consent.

The personal data will be deleted after four years. You can ask for the personal data to be deleted at any time.

Who will get access to the personal data?
